Dear CPSE and CSE Chairs,
In an effort to work more collaboratively with school districts in facilitating referrals, and in light of the fact that many of you are new to your positions, we thought you would find the information below useful with regard to our application and screening process. As part of our obligation as a licensed mental health facility, we need to insure that the students we admit meet the criteria for day treatment services. Specifically, they must have a documented mental illness diagnosis and their mental health needs must be the primary interference in their educational progress. In order to responsibly evaluate applicants according to these criteria, it is required that referral material, and, in particular the assessment materials, be current (within 1 year of referral date) and complete for us to proceed with our evaluation as follows:

  • Psychological Assessment
  • Educational Assessment
  • Social History
  • Psychiatric Assessment
  • Physical and Immunization Records

Like all of you, we know that the children and families referred to us have experienced many disappointments and a great deal of uncertainty; and, that you shoulder much of the burden in buffering that experience for the children and families you refer. It is our hope that in clarifying our requirements in advance, we can help simplify the application process–for you and for them–and lessen some of the stress that is an inevitable part of the search for a program that can ably meet the needs of some of the most vulnerable students you serve.
